Lake Forest, IL – If your child is starting early orthodontic treatment, you probably have a few questions. One common question is “If we complete phase one at a particular orthodontist’s office, does that obligate us to complete phase two there as well?” Dr. Michael Stosich answers that question.
“The answer is most certainly no – you are never obligated to return to the same office for phase two,” says Dr. Stosich. “I see a lot of families who did phase one at one office and believe they have to go back there for phase two. That just isn’t the case. Your child’s phase two treatment doesn’t have to happen at the office where phase one was done.”

What is two-phase orthodontic treatment?
Two-phase treatment is designed for kids who have orthodontic issues that benefit from early intervention.

Phase one, sometimes called interceptive treatment or growth guidance, usually begins between the ages of 7 and 10, while your child still has a mix of baby and permanent teeth. During this phase, Dr. Stosich uses orthodontic appliances such as palatal expanders to guide the growth of the jaw. This is also when bite issues like crossbites and underbites can be corrected, making room for the permanent teeth that are still on their way in.

After phase one is complete, there is a resting period. This gives the remaining permanent teeth time to come in naturally. Phase two usually begins in the early teen years, with full braces or clear aligners, and it fine-tunes how the teeth fit together so your child ends up with a healthy bite and a beautiful smile that lasts a lifetime.
“Phase one does part of the work while the jaw is still growing, and phase two finishes the job,” says Dr. Stosich. “But not every patient needs two phases – many kids, and most adults, are treated in a single course of treatment.”
So, are you obligated to stay for phase two?
No. Phase one and phase two are typically treated as two separate treatment plans, each with its own fee and its own agreement. Once phase one is complete and paid for, your commitment has been fulfilled. Where your child receives phase two treatment is completely up to your family. There is no penalty for going elsewhere, and no negative effect on your child’s care. Any qualified orthodontist can review your child’s records and continue right where phase one left off.
“Parents sometimes worry that asking about switching offices will be an awkward conversation,” says Dr. Stosich. “It shouldn’t be. A quality orthodontist wants what is best for your child, and that includes respecting your family’s right to choose. Your child’s records belong to you, and any office should hand them over promptly when you ask.”
When switching offices might make sense
That being said, there are times when changing providers between phases is the right call. These can include:
- Relocation. If your family is moving away from the area, continuing care locally just isn’t practical.
- Insurance changes. A change in your dental insurance plan might mean another office fits your coverage better.
- Comfort and confidence. If you weren’t happy with your phase one experience, or you felt like your questions weren’t being answered, you deserve a team you feel comfortable with.
- A better orthodontist in the area, or a second opinion. Phase two is often the more complex phase, and there may be an orthodontist nearby whose experience and credentials are a better match for what your child needs. Even a simple second opinion can point you to a better plan. Wanting the best care available is always a valid reason to switch.
If you do decide to make a change, just ask for a copy of your child’s records and any progress notes. A reputable office will hand them over without any fuss.
And even if you were happy with your phase one experience, it is worth doing your homework again before phase two begins. There may be new providers in your area since your child started treatment, and your family’s needs may have changed too. Phase two is a separate treatment plan, so treat choosing where to do it as a fresh decision. Schedule a consultation or two and compare your options before you commit.
A second opinion is especially valuable at this stage. Not every orthodontist will recommend the same phase two plan, or even the same time to start it, and hearing from more than one expert either confirms the plan in front of you or points you to a better one. What should you watch out for before starting treatment?
Unfortunately, not every office approaches orthodontic care the same way. There is a name in the industry for practices where making production takes priority over patient care: a “dental mill.” Whether your child is starting phase one or phase two, here are a few warning signs to be aware of:
- You never meet the orthodontist. If the idea that your child needs orthodontic treatment is presented by a treatment coordinator or a sales representative, but you never actually have a consultation with the orthodontist, that should give you pause.
- Records taken without your consent. Orthodontic records should never be taken at a regular dental visit without your knowledge and permission.
- Pressure to start treatment. A treatment recommendation should never feel like a sales pitch. If you are being pushed to commit before you feel ready, take a step back.
